Townsville Community Coordinated Pilot

Open Arms is exploring whether a new model of community-coordinated care will enhance the service experience of clients in the Townsville region. This pilot is intended to improve the care for clients with complex issues, or who are considered to be at risk of suicide.

The new Care Coordination team comprises a skilled Open Arms clinician and a lived experience peer, working together to support more comprehensive and integrated community care for clients.

The pilot will run over two years, in four phases. Each phase will provide or the benefits of the pilot to be identified, be evaluated, and the model adjusted where required.

Open Arms wants to find out whether this model of coordinated clinical and peer care is suitable for wider implementation.
