Townsville Community Engagement Pilot (North Queensland region)
Open Arms trialled a new model of community-coordinated care that enhanced the service experience of clients in the Townsville region. This pilot was intended to improve the care for clients with complex issues, or who were considered to be at risk of suicide.
About the pilot
The new Care Coordination team comprises a skilled Open Arms clinician and a lived experience peer, working together to support more comprehensive and integrated community care for clients.
The pilot was run over two years, in four phases. Each phase provided a range of benefits that were identified and evaluated, and the model was subsequently adjusted where required.
